The Contest: The MASTER JEWELERS GET LUCKY COLTS EXPERIENCE (“Contest”) is sponsored by Fine Line Jewelers, Inc. d/b/a Master Jewelers (“Sponsor”). From 12:01 a.m. on October 1, 2015 through 11:59 p.m. on October 31, 2015 (the “Contest Period”), Indianapolis Colts, Inc. (“Club”) will accept entries to win a comprehensive game day experience (the “Grand Prize”) as detailed below.

Eligibility: Each entrant must be 18 years of age or older and a U.S. Citizen with a valid driver’s license to participate. Residents of Rhode Island or Puerto Rico are not eligible to enter. Any individuals (including but not limited to employees, consultants, independent contractors, and interns) who have, within the past six months, performed services for Sponsor, Club, the National Football League (the “NFL”), or any organization responsible for sponsoring, fulfilling, administering, or promoting the Contest, or supplying the prize, or who won any sweepstakes or prize administered by Club, or any of their respective parents, subsidiaries, affiliates and successor companies, and immediate family and household members thereof, are not eligible to enter. “Immediate family members” means parents, stepparents, children, stepchildren, siblings, stepsiblings, or spouses. “Household members” shall mean people who share the same residence at least three months in the preceding one calendar year.

By participating in the Contest, each entrant unconditionally accepts and agrees to comply with and abide by these Official Rules and the decisions of Club, which shall be final and binding in all respects. By participating in the Contest, participants agree to release, discharge, and hold harmless Sponsor, Club, and their respective affiliates, subsidiaries, divisions, retailers, agencies, officers, directors, employees, and agents, from any and all damages whether direct or indirect, which may be due to or arise out of participation in the Contest or any portion thereof, or the acceptance, use, misuse, or possession of prize.

How to submit entry for the Prize: Entries for award of the Grand Prize will be accepted during the Contest Period. Eligible participants may enter the Contest online at: https://www.colts.com/masterjewelers

Multiple submissions may result in disqualification. Illegible or incomplete entries are void. Club is not responsible for lost, late, delayed, mutilated, misdirected entries or entries not received by the entry deadlines.

Prize: One Grand Prize random drawing will occur on November 2, 2015. Grand Prize includes two tickets to the Indianapolis Colts vs Denver Broncos, two pre-game sideline passes, limo ride to and from the game, one autographed item, one $50.00 Colts Pro Shop gift card, one $100 St Elmo Steakhouse gift card, a $1,200.00 shopping spree at Master Jewelers, and $291 silver tag pendant. The approximate retail value of the Grand Prize is $2,000. Drawing for the prize will be made from all Entrants. The drawing will be conducted by Club. If the prize winner does not meet Official Rules or eligibility requirements, or he/she forfeits the prize, Club will award the prize to another winner selected in the same manner. Club shall not be responsible or liable if a prize winner is unable or fails to utilize a prize. Odds of winning depend on the number of entries received.

Prize is non-transferrable. There may be no cash or other substitutions of prize by winner. However, Club reserves the right to substitute a similar prize of equal or lesser value in the event the above prize is unavailable for any reason. All costs and expenses associated with the acceptance and use of the Prize not specified herein as being awarded, including, but not limited to, all federal, state and local taxes, license and registration fees, any ground transportation, and miscellaneous travel expenses, including, but not limited to, airline luggage charges, are each winner’s sole responsibility.

Prize winner(s) will be required to sign an Affidavit of Eligibility, Release, and Indemnification Form that: a) releases, indemnifies and holds harmless Club, the NFL, Sponsor, and their respective parents, affiliates, subsidiaries and successor companies, and their respective officers, directors, employees, agents, and representatives, advertising agencies and promotional sponsors from and against any and all harm, expense, loss, damage, liability, or injury resulting directly or indirectly from participating in the Contest or awarding, receipt, possession, use, or misuse of any prize awarded in the Contest, b) contains the winner’s agreement to be bound by Contest official rules, and c) authorizes Club to use the winner’s name(s), likenesses, photographs, voices, and biographical data for advertising, promotional, and any other purpose, in any media. If a prize winner fails to sign an Affidavit of Eligibility, Release, and Indemnification Form within ten business days of receipt, the prize winner may be disqualified and an alternate prize winner may be randomly selected from the remaining valid entries.

General Rules: Prize will not be released until Club receives all required information. Failure of prize winner to provide all required information within the required timeframe will result in automatic forfeiture of any prize and such prize shall be awarded to an alternate winner. In the event of a prize forfeiture, the winner or parent or legal guardian of the winner forfeiting prize will sign a Prize Forfeit Form and return the form to Club within ten days of receiving the form.

Unforeseen Events: Club reserves the right to change this Contest and the Contest rules at its sole discretion at any time that Club becomes aware of any circumstance or occurrence which would materially affect the Contest’s security or fairness. Club reserves the right to announce any Contest rules changes on its website, over the air, in print or posters. Club is not responsible for the inability of the winner to accept the prize for any reason. Club reserves the right to substitute different prize elements of approximately equivalent value at its sole discretion.

List of Winner and Official Rules: For a copy of the winner list and Official Rules, send a self-addressed, stamped envelope to: Master Jewelers Sweepstakes Entry, c/o Laura Porter, Indianapolis Colts, 7001 W. 56th St., Indianapolis, Indiana, 46254. The letter should specifically request a copy of the “winner list” and/or “Official Rules.” Such requests must be received between October 5, 2015 and November 30, 2015.

Law: These Official Rules are governed by and shall be interpreted in accordance with the internal laws of the state of Indiana. By entering, each participant, along with Club, irrevocably and unconditionally consents to the exclusive jurisdiction of the courts of the State of Indiana and of the United States of America located in Marion County, Indiana. Any controversy or claim relating to the Contest and/or these Official Rules shall be submitted to arbitration before the American Arbitration Association in Indianapolis, Indiana under its Commercial Arbitration Rules. Judgment upon the award rendered by the arbitrator may be entered in any court of competent jurisdiction. If any portion of these Official Rules is found to be invalid, illegal or unenforceable, for any reason, that portion shall be severed from the rest and shall not affect the agreement to resolve all controversies and claims through arbitration. Claims and causes of action arising out of or connected with this Contest, or any prize awarded, shall be resolved individually, without resort to any form of class action, and exclusively by arbitration. Claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Contest, but in no event attorney’s fees or any other damages of any kind. These Official Rules supersede any and all agreements or communications of any kind between participant and Club or anyone associated with the Contest, and cannot be modified or waived except in writing and unless properly signed.
